Patten Academy of Christian Education is an educational ministry of Christian Cathedral church and seeks to provide the best in academic preparation in a Christian environment that will nurture the ethical and moral development of your child and prepare him or her for the future.

1. *Student application to be filled out completely, signed, and dated by parents or guardians. K-12 Application Form (2023-2024)
   -for students enrolling in kindergarten, please complete the Kindergarten Information Sheet
   -for I-20 student application, please complete the I-20 Application Form (2023-2024)
2. *Statement of Purpose: Student (Grades 6-12)
3. *Recommendation Forms (Grades 6-12), they have to be signed and stamped
4. *Pace Chromebook, Google Workspace, and E-rate Forms 2023-2024, please sign and complete pages 5, 6, and 10.
5. **A copy of the applicant’s most recent report card and test scores.

8. **A Doctor’s Report of Health Examination for School Entry
9. **Immunization record; please refer to the CA Immunization Requirements K-12th Grade for details.
10. ***Application fee of $50.00 ($100.00 for I-20 applicants) cash, money order, cashier’s check, ATM or credit card (Visa, MasterCard, AMEX and Discover). NO personal checks, please.
